    We are taking our 6 yr old grandson with us on the Disney Fantasy in about 3 weeks and our travel agent mentioned there was a form his parents needed to fill out to allow us to take him but I cannot find it on the website. Can you help? Thanks!!!

    Welcome aboard, Judy! I bet you have one very excited six-year old grandson right now! What a treat for him to get to not only sail on the Disney Fantasy, but to have his grandparents all to himself! You're all in for one very magical cruise!

    To access the Minor Authorization From, log into your reservation on Disneycruise.com. Then, click on "My Online Check-In", where you can scroll to the bottom of that page and see "Optional Forms". The Minor Authorization Form is on the bottom right. Simply download the PDF file and bring the signed form with you when you check-in at the port terminal.
