When my family visited Disneyland a couple of years ago, we began our vacation in San Diego and then made our way up to Anaheim in a rental car. It took us about two hours with very little traffic (we left around 10:00 a.m.). I did a little research for you and found out there is an Avis Rent A Car location near the port that does pick-up cruise passengers. There is also one not too far from Disneyland. If you decide to go this route, you will want to double check on their policies on one-way rentals, and if they will provide a shuttle from their West Patella Ave. location in Anaheim to your hotel. The other option I found for you is hopping on an Amtrak train at the Santa Fe Depot Station and taking the scenic route up the Pacific Coast to Anaheim. From what I researched, the Pacific Surfliner route is about two hours long, and they do provide a shuttle to Disneyland once you arrive in Anaheim. The Santa Fe Depot does not appear to be too far from the port (about 0.6 of a mile) so you can take a taxi if you don't want to haul your luggage on foot.
My family is looking forward to our second trip to Disneyland this year for the Diamond Anniversary Celebration!
