    Where can I get a parental consent form for a child coming to Disney with us?

    Hi, Sherry!  You can find a standard parental consent form by conducting your own Google search.  Just search for parental consent to travel, and you'll find links to a standard form.  The form has to be filled out by the minor child's parents, and then signed in the presence of a notary public. 

    Other items you may want to bring with you include a copy of the child's health insurance card, just in case you need it for an emergency.  Try to find a form that will allow you to seek medical treatment in the absence of the parents.  Some of the standard travel consent forms will have it, but you may have to print out a separate form for consent for medical treatment.
