At some character meals a photograph is part of the dining package, and at many others pictures are taken, but not part of the package. In the second example they bring prints of the pictures by your table and you can choose to buy them or not.
As for attractions, there are many where they take your picture. You will see them as you exit the ride. You can buy them if you want, but at a cost.
In none of these cases were the photos put into your PhotoPass account. HOWEVER in 2012 Disney introduced PhotoPass+. With this enhanced version of PhotoPass you get select dining and attraction photos added to your PhotoPass account. The list can change, but at last word it included these venues;

I recommend you go here before your trip to pre-purchase PhotoPass+ if you are interested.
Disney has also announced a new program called Disney Attractions+. This covers just the attraction photos, for a lower added cost. Obviously you will want to choose by comparing how many of the character meals are in your plans.
