Disney Resorts Collection hotels,
theme park tickets are not automatically included in the room price itself. However, the good news is that you absolutely can book a Walt Disney World vacation package instead. This is usually the easiest option because it bundles your hotel stay and your theme park tickets together into one reservation. You'll see the total price upfront, which makes budgeting and planning much simpler and far less stressful.
Now, as for Lightning Lane passes, these are purchased separately through the My Disney Experience app. Lightning Lane Multi Pass allows you to reserve access to select attractions with shorter wait times throughout the day, while Lightning Lane Single Pass is used for certain individual attractions that are not included in Multi Pass. The pricing for both options changes constantly and depends on several factors, including the park you're visiting, the date of your visit, the season, and overall demand. Because of that, there really isn't one set price I can confidently give you ahead of time, especially considering how far in advance you're planning. Please note, however, that you will not be able to book anything for 2030 until 2029. That means you still have over THREE years to budget and plan!
Since you mentioned health issues and difficulty standing in long queues, I do think Lightning Lane Multi Pass could be incredibly helpful for your trip. It can make park days feel much smoother and less physically exhausting, especially during busy seasons.
