I had to do a little research on this one -- and it's research I will now put to good use myself the next time I'm in the parks :-)

The in-park locations I have found that serve frozen Coke are Scuttle's Landing and the Lunching Pad in the Magic Kingdom and Club Cool at Epcot. There may be other locations as well. Frozen Coke can be purchased using a Disney Dining Plan snack credit. At Club Cool (for an additional non-DDP fee) there is a station where you can create your own character themed mug for your frozen Coke.

And you're right about the Pop Century. At this WDW resort only, frozen Cokes (and other frozen Icee flavors too) run on tap in the beverage dispensing area. They can be purchased individually or can be used to fill your refillable mug. Frozen Coke on tap! Mmmmmmm.
