So here is what I would do, based on your notes;

October 11 - Magic Kingdom
October 12 - Epcot
October 13 - Magic Kingdom
October 14 - Hollywood Studios
October 15 - Animal Kingdom
October 16 - Magic Kingdom
October 17 - Departure Day

I was not sure if you had time for a park on your departure day. If so, I would go to the Magic Kingdom that day, and go to Epcot on the 16th. I love going to IllumiNations on the last night!
