From the 2010 Dining Plan Brochure, am I correct that it basically states the gratuities are not included for any Table Service restaurant when using the Dining Plan EXCEPT for CRT, the Dining shows and pizza service, all of which require 2 credits?

Yes, that is correct. When using the Disney Dining Plan, for the vast majority of dining experiences, the gratuity is not included. You will need to leave your own tip at the end of the meal. As you mentioned, there are a handful of exceptions to this rule, including Cinderella's Royal Table, the dinner shows, and in-room dining. The gratuity-included meals are all indicated on the Disney Dining Plan brochure.
