    Daughter's 16yo friend is going on cruise with us (I have minor auth form, birth cert and license). Her mother's last name on BC is different than her current last name on minor auth form (remarried). Will this be an issue?

I’m glad you decided to stop by with your question about the Authorization for Minor to Travel without a Parent or Legal Guardian Form. Let’s ensure you are shipshape and ready to sail on your upcoming cruise on Disney Cruise Line. 

    The Authorization for Minor to Travel without a Parent or Legal Guardian Form is required for every Guest that is traveling with a child when they are not the parent or legal guardian. I’m happy to hear you already have the form completed along with a copy of the child’s birth certificate and license. Although the mother’s last name is now different than what is listed on the child’s birth certificate, as long as you have her signature with her current legal name on the Authorization for Minor to Travel without a Parent or Legal Guardian Form, and you bring the original with a wet signature (not a copy), you will be ready to set sail. There is no issue with it not matching the mother’s last name on the birth certificate.
