Authorization for Minor to Travel without a Parent or Legal Guardian form. The form must be completed by the child's parents or legal guardians and presented, along with the child's
government-issued identification, by the designated adult during check-in at the port terminal. Remember, when you disembark the ship at ports of call, you need to have the child's identification and Key to the World card with you.
