Disability Access Service is for Guests who cannot wait in a traditional queue. You do not need to bring any proof to the parks to obtain this; simply stop by City Hall in Disneyland Park or the Chamber of Commerce in Disney California Adventure Park and speak to a Cast Member about your needs. They will discuss the different accommodations available and register you for Disability Access Service if deemed appropriate. You can also obtain the tag to use your stroller as a wheelchair here. Since these are two different options for Guests with disabilities, you should be able to use both, but the Cast Member at Guest Relations will be able to confirm.
