You don't want to be figuring this out when you wake up on the day and you're still wiping sleep from your eyes.
There are a couple of convenient options for you. The first is to take a shuttle from your hotel to the Transportation & Ticket Center (TTC), and then either walk over to
Disney's Polynesian Village Resort or take the Monorail (the "Poly" is the first stop on the resort monorail). Alternatively, a rideshare service like Lyft or Uber, or the Minnie Van Service, is also available if you want the fastest, most direct route.
Pro tip: Aim to leave your hotel at least 45 minutes before your reservation to allow yourself plenty of time to make your reservation. If you arrive early, enjoy walking around Disney's Polynesian Village Resort.
