When I've requested a Disability Access Service Card (DAS) for my son, it was different for each park. At Disney's Hollywood Studios, we were escorted to a table just outside Guest Relations. We felt very comfortable discussing my son's needs there with the Cast Member. At Magic Kingdom, it was inside Guest Relations. This may vary depending on how busy they are at Guest Relations when you visit.
When you are greeted by a Cast Member at Guest Relations, I would recommend that you tell them you are requesting a DAS but would like to discuss the reason in a private or remote area because it involves a sensitive matter. They have always used an iPad mini when taking my son's picture and information so they should be able to accommodate you. All of the Cast Members we've spoken with have been really great and understanding when assisting us.
