My mother-in-law is taking my son on a Disney Cruise next year, so I’ve done a bit of research on the
Authorization for Minor to Travel without a Parent or Legal Guardian form. If both parents share joint custody of the minor, both must sign the form. On embarkation day, you’ll present this form, your Port Arrival Form, and everyone’s travel documents.
According to
Disney Cruise Line’s Travel Documentation page, US citizens heading out on a closed-loop sailing—meaning your cruise begins and ends at the same US port—have a couple of options for identification. You can bring an original valid US passport or an original state-issued birth certificate along with a physical government-issued photo ID. Children 15 years old and younger don't need to worry about a government-issued photo ID.
