Yes, the
Authorization for Minor to Travel without a Parent or Legal Guardian form is required for every guest that is traveling with a child when they are not the parent or legal guardian, no matter their home country. There is no different form for guests that live outside of the United States, so please use the one that can be accessed on the Disney Cruise Line website for your travel party. You may use a notary from your country, or from the United States, whichever is more convenient for you.
The form lists space for two minors, however, additional minors with the same parents or legal guardians may be added underneath if necessary. Additionally, only one parent or legal guardian is required to sign the form.
I hope that clears up your question! I just recently had to fill out this form and get it notarized so that my son could sail to Alaska with his great-aunt. I made an extra copy of the form to keep at home with me, and a second copy so that both my aunt and son could have one in their carry-on bags for safekeeping.
