The “Authorization for Minor to Travel without a Parent or Legal Guardian” form can be found
here. It is only required for children who are traveling without a parent or legal guardian, so that there is documentation officially designating the adult (age 18 and older) who will be caring for them during the sailing with Disney Cruise Line. The form provides the designated adult with authority to access information and make decisions for the minor in the case of an emergency, but also allows them to sign any consents or waivers needed for the minor to participate in certain activities, such as
Port Adventures. Further details can be found in the Cruise Contract, which is available

When you complete your online check-in, you must provide information on all the members of your sailing party, so make sure to have all of the necessary
travel documents and a recent headshot photograph for the minor traveling with you. Additionally, be sure to bring the signed authorization form with you when you check-in in person at the port.
