Although it's not a requirement, some restaurants will look to add something "extra" if they know it's someone's birthday. I've seen some put a little confetti on the table beforehand, and others give a dessert such as a cupcake with candle at the end of the meal. I must note, that this is all at the restaurant's discretion.
The two where I've actually seen them stop and sing "Happy Birthday!" to a guest was at the Whispering Canyon Cafe' at Disney's Wilderness Lodge and 50's Prime Time Cafe' at Hollywood Studios. But I'm not sure if this was standard protocol or just one of those special moments Disney is so good at creating. Many times, it depends on your waiter or waitress. And perhaps they sang at those 2 restaurants because the ambiance is fun, silly, and a little rowdy :-)
