Welcome aboard! A magical cruise with a grandparent sounds like the work of a fairy godmother. I'm sure your grandson is so excited to go on an adventure with you!
For kiddos under the age of 18 to sail without their legal guardians you already know you'll need the
Authorization for Minor to Travel without a Parent or Legal Guardian form. That form is to be filled out by the legal guardians of the child naming you as the "Designee" who will be responsible for them during the cruise.
Taking a look at the current version of the form, I believe that the top section where the name of the child and the name of the Designee is provided should include your address as part of the Designee's information. The bottom section where each guardian signs to give consent should include the name, address, and phone number of the guardians who are allowing you to sail with their child. So both your address and your grandson's will be on the form, yours first and his at the end.
