Specifically, you're going to want to utilize
First Aid locations in the theme parks, as they have restroom facilities that provide additional space and privacy for individuals who may need personal care assistance from a member of their party. These locations offer a dedicated adult changing table. You can find a list of where these centers are located on the page I linked above, and also in the Disneyland Resort app's map page.
Additionally, if it's helpful, there are also
companion restrooms located throughout the resort as well.
As you continue to plan your trip, I would also highly recommend the
