As a breast cancer survivor myself, let me first say congratulations on making it to the end of your treatments--and what a way to celebrate by going to Disney World! Unfortunately, there are no meal plans available at this time for guests staying off property.
Having visited both Disneyland and Disney World shortly after my chemo and radiation treatments, I can echo your thought that you may be tired and your stamina far less than imagined. However, thankfully, you'll be there in November and not under the heat of summer that can further zap your energy. Although I did not need a
wheelchair, they are available at every park if you need them. Just keep in mind that you cannot take them from the theme parks. If you have a hunch you'll need one at your off-property hotel, you may want to look at renting one for the duration of your trip. Disney's
list of recommend area rental agencies can help.
Even though you are staying off property, I'd also recommend leaving the parks after lunch for a good nap and then returning in the evening. It may sound inconvenient, but what is worse is having to stay in the bed on Day 3 of your vacation...I took a long nap every day and was
still tired.
