    We will be in DW in 3 weeks, Mom,Dad,16 year old and her 16 year old friend!!! What kind of forms should I have from the friend's father? I would guess permission to travel with us,stay with us at DW,maybe medical info? Any pre-printed forms available?

    We brought my daughter's BFF along on vacation with us a few years ago, Kathy.  While I did fret a bit over the formalities, everything turned out fine.  We brought a notarized letter from the child's mother.  It was not pre-printed; her mother just wrote a brief statement authorizing us to travel with her daughter and obtain medical care for her if necessary.  We also brought along a copy of her insurance card just in case.  Turns out we didn't need it.  Our trip was blissfully smooth and I hope yours will be too!
