In my travels this past weekend, I was with two friends that both use the
Disability Access Service pass (DAS) during their Walt Disney World Resort vacations. I learned a lot watching the process and am happy to help you with your question! Having an official doctor's note will help the process when you are speaking with the Cast Member at Guest Relations. It doesn't have to be anything fancy or in great detail! Just something that explains your disability and why you are unable to wait in line.
When you arrive on the first day of your visits to the theme parks, head straight to
Guest Relations near the main entrance to speak with a Cast Member and present your doctor's note. You can find the list of specific locations at the Disability Access Service link I included above. When they issue your DAS pass, it will be good for your entire vacation, so you only need to visit Guest Relations once!
