You have a very good question. To answer simply, you do not need any documentation to receive a Disability Access Service Card (DAS).
The DAS card is designed to accommodate Guests who are unable to wait in a conventional queue environment due to a disability. In order to receive your DAS card, go to Guest Relations at the main entrance of any theme park. There you will register with a Cast Member, have your picture taken for the card, and have any questions that you might have answered.
