    We're booking two rooms (1 adult and 2 kids per room), but are thinking of only getting the meal plan for one room. Is there any issue with using two adult table service meals at the same time from one card (that has "1 adult 2 kids" on it)?

     I believe the rules regarding the dinning plan state that you are not allowed to buy a meal for someone who is not on the plan.  You can go to a restaurant and pay for one meal cash and use the dining plan for the other. 

    It might not be the answer that you wanted, but those are the rules.
