Hey, I always love it when Guests are on the ball as far as their travel documentation is concerned. Nobody wants a surprise at the port! The
Authorization For Minor To Travel Without a Parent or Legal Guardian Form, allowing an adult other than the parent or legal guardian to retain care, custody and control of a child during a Disney cruise, does NOT need to be notarized. As you know,
this form is required in situations where a child is traveling without a parent or legal guardian. If you look at the form, it only has a signature line for the parent/legal guardian and no indication that a notary has to witness and sign this particular form. So it’s not a requirement that the form be notarized.
I think I know why you are wanting to clarify this point since I mentioned notarizing the other day in an answer. Sorry if that made things more confusing. But I was talking about if a Guest also has a separate document known as a Minor Travel Consent Form. Since you will be submitting the Disney Cruise Line authorization form linked above, you have it covered as far as Captain Mickey is concerned.
